Health Equity WG: SDOH Code Systems and Screening Tools Resource

The SDOH Code Systems and Screening Tools Resource is a list of social determinants of health (SDOH) code systems and screening tools that are commonly used today for collecting and exchanging SDOH data. The intent of this resource is to raise awareness and increase use of these code systems and screening tools to improve standardization of the data.
